Program participation

Client acknowledges orientation and treatment consent, agrees to participate in the PHP treatment plan and program rules, and is told that services may include group and individual therapy, case management, assessment, screening, and post-treatment planning.

Payment and insurance responsibility

The basic fee is due when the agreement is signed. The client or payor is responsible for deductibles, copays, coinsurance, uncovered services, third-party charges, and costs if insurance authorization or coverage is denied. The packet also describes a late fee after two weeks and possible suspension of services or collections for unpaid balances.

Refunds and early exit

The forms state a general no-refund policy and say early departure may leave the client or payor liable for the full agreed term less amounts paid. Leaving against clinical advice or discharge for rule violations does not qualify for a refund. If CAST decides it cannot provide appropriate treatment for reasons outside the client's behavior, it may consider a refund case by case, subject to a 20% administrative fee if approved.

Conduct and testing

Rules cover attendance and punctuality, staff direction, prohibited violence, sexual behavior, drugs and alcohol, and no cameras or recording in sessions. Noncompliance may result in discharge. Client consents to random drug and alcohol testing throughout services, laboratory sharing of results, and payment of testing costs not covered by insurance, including out-of-network lab costs. The testing form also contains a waiver and indemnity concerning test results, including inaccurate results.

Client confidentiality

Client agrees not to disclose other clients' identities, participation, fees, or information about their families and associates. The admissions agreement separately restricts publicity about its terms and negotiations, past or current clients, and CAST program techniques without written consent unless legally compelled.

Information sharing

The packet authorizes treatment-related disclosures and insurance claim information sharing, including records from other providers to CAST. It includes a privacy notice describing treatment, payment, and operations uses and patient record rights. Separate forms permit non-client participation in groups and post-discharge contact; the follow-up consent says it can be revoked.

Insurance claim authority

Client appoints CAST as attorney-in-fact and authorized representative for claims and appeals concerning CAST services, assigns insurance payment rights and certain causes of action, authorizes direct payment, and agrees to forward insurance payments received personally by the client. The form describes possible administrative and legal action against insurers or benefit plans.

Disputes and liability

The admissions agreement calls for American Arbitration Association arbitration of contract disputes in Los Angeles under California law before one arbitrator. It also contains broad release and indemnity language for injury, property damage, wrongful death, and claims connected with services; the confidentiality and testing forms contain additional waivers.
